This charming little orchid is a woodland native in the eastern U.S. I purchased it from a small nursery that propagates native wildflowers. Not many places carry really unusual plants like this. Be especially wary of the ones that have unusual plants that they sell for a surprisingly low price. Chances are they were dug from the wild. The large twayblade grows in shady woodlands in loamy soil. Mine was about six inches tall last year but it can reach ten inches in size.
